To better serve you, we ask that you print and complete the listed patient forms prior to your appointment. In addition, please bring any medical records, x-rays/MRI scans (and reports) or other test results with you to the appointment.
Please try to arrive early so that you are ready to see the doctor at your scheduled time. If this is your first visit, we recommend that you arrive at least 5 – 10 minutes prior to your appointment time so that all necessary information can be gathered.
In case you are unable to keep your appointment, please notify us as soon as possible, preferably 24-hours in advance. We do maintain a cancellation list for patients who may desire to use these appointment times. There is no charge for cancelled appointments, but the right to do so is reserved in cases of repeated cancellations.
Dr. Kramer does not participate with insurance plans. However, since some insurance companies may provide reimbursement for certain services, you will be provided with a receipt containing the necessary information should you choose to file a claim. This means that we do not bill insurance companies and expect payment to be made at the time of service and request that you come prepared to do so.
Because your privacy is important to us, we do not have your medical records online, nor are they released without your written consent.
